Thank you for choosing us to help you with your upper extremity problem. For an urgent or emergent appointment, please call us directly at 617-738-0857.

To make an appointment you may call the office (617-738-0857) or for non-emergent or non-urgent appointments you can start the scheduling process by filling out the new patient form and faxing it to our office (617-738-0857) or emailing it to us.

To expedite your visit there are a number of other forms that can be to be filled out prior to being seen. These include the health questionnaire, hand diagram and DASH questionnaire. You may print the forms, fill it out and fax it (617-731-3109) or bring it to your first appointment. Alternatively the health questionnaire and DASH questionnaire can be emailed to us after filling it out.

At this time, the hand diagram cannot be filled out on line. Please follow the instructions at the top of the diagram. Please fill it out and fax it (617-731-3109) or bring it to your first appointment.

Please make sure to have the appropriate referral with you. Please arrive about 15 minutes prior to your scheduled appointment to allow time for parking, finding the office and any last minute problems. We will do our best to stay on time, however medical emergencies do occur; therefore, please let us know as early as possible of any time constraints.

Please bring the following that relate to your problem. This will help us fully evaluate you:

  • All previous medical records
  • All previous x-rays (include CAT scans & MRIs), reports, and actual images
  • Previous splints used or a description of the splint
  • Referral
  • Insurance card and information
  • Email, or send or bring completed health questionnaire, hand diagram and DASH form.
  • Photo ID

We have onsite x-ray and therapy to help provide comprehensive care. Emergent CAT scans and MRI’s can often be done the same day. So please plan accordingly.
