We participate in most insurance plans and will submit claims to all health insurance plans. However, it is your responsibility to make sure that our physicians are providers with your insurance plan. You are also the one to know best what is covered and what referrals are needed for your plan. Contact your insurance company with any questions.
Please bring your insurance card to your first visit, as we will need to make a copy of it. Also if your insurance requires a referral please make sure that your PCP has issued the referral before your appointment. If you do not have a referral you will be financially responsible for the visit. All co-payments must be paid at each visit.
If your visit is to be covered by Workers Compensation, we will need the claim number, the name and phone number of the adjuster, and the name and address of the insurance company. Please make sure you have this information when you schedule your appointment.
If you are involved in a motor vehicle accident, each visit must be paid for at the time of the visit. We do not accept liens and we do not bill the PIP insurance or Attorneys. If your PIP is exhausted we will bill your health insurance if you bring in a letter of exhaustion from your PIP carrier. However, when billing your health insurance you must obtain a referral from your PCP if your insurance carrier requires it.
Payment is required at the time of service.
